SAMPLE POLICY FOR EMPLOYEE REPORTING OF HAZARDS
Every employee is expected to watch for and to report any possible hazards to employee safety and health. You may make your reports by speaking to your supervisor or by submitting a written report through the
(use your company's name for the written hazard reporting system). Make your report immediately or as soon as possible.
No employee, at any level, shall discipline or harass any other employee because of reports made of hazards. Any employee found to have discriminated against another employee for this reason shall be disciplined.
Remember, YOU are needed to help keep this worksite safe and healthful!
